Policy on the Collection, Use and Disclosure
of Personal Information of Parishioners

St. Andrew Roman Catholic Church (“St. Andrew”), as a non-profit charitable organization, is committed to enhancing the spirituality of its parishioners. Our Mission states that we are called to be apostles of Jesus Christ; to celebrate the sacraments and proclaim the word of God; to give witness to our faith in our daily life; and to share our talents in building a Catholic community of love. With this in mind, St. Andrew is dedicated to maintaining high standards of confidentiality with respect to the information that has been provided to us. This Policy Statement has been prepared to affirm our commitment to maintaining the privacy of our parishioners and others and to inform you of our practices concerning the collection, use and disclosure of information provided to St. Andrew.

Our obligations apply to all priests, employees, volunteers, contractors and agents, who provide services to or on behalf of St. Andrew in connection with ministering to our parishioners. Other applicable laws and internal policies govern the protection of personal information of employees and volunteer staff of St. Andrew.

St. Andrew collects information about parishioners through its general registration form and does so to fulfill the objectives and mission of the church. The achievement of St. Andrew’s mission involves, among other things, communicating with and determining the needs of its parishioners and ministering to them. St. Andrew also collects information about parishioners through registration forms for participation with a variety of ministries and services.

As a general rule, all parishioner information is held in strict confidence and, except in limited circumstances, is not revealed to anyone unless expressly or implicitly authorized by the parishioner. It is understood that parishioner information is not and will not be provided to third party organizations for solicitation purposes.

There are circumstances where disclosure of personal information may be justified or permitted under a legal duty or right and St. Andrew may disclose parishioner information without consent. Personal information may include all information about parishioners collected on the registration form. In such circumstances where, in the opinion of St. Andrew, it is appropriate or necessary to disclose personal information, St. Andrew will not disclose more information than is required.

St. Andrew will retain personal information provided to it only for so long as it is required for the purposes stated above.

St. Andrew endeavors to maintain the accuracy of any information about parishioners in its possession. Information about a former parishioner is not actively maintained and, for so long as it is held by St. Andrew, St. Andrew cannot assure the accuracy of such information.

While specific responsibilities may be assigned to other members of St. Andrew staff, including volunteer staff, the Screening Committee is responsible for the administration of this Policy. In the event of any question concerning the access to or the accuracy, management or use of a parishioner’s personal information, parishioners should address such questions to the Screening Committee.

St. Andrew endeavors to maintain adequate physical, procedural and technical security with respect to our office and information storage facilities so as to prevent any unauthorized access, disclosure, copying , use or modification of a parishioner’s personal information.

Visiting St. Andrew Web Sites

A visitor to St. Andrew Web site is not required to reveal any individually identifiable information, such as name, address or telephone number. Nor is such information collected passively by electronic means.

Information is collected when an individual voluntarily completes an online registration form or completes an online survey. This information is collected, used or disclosed in a manner consistent with this Policy Statement. Email addresses are also collected during the online registration process, but parishioners may indicate that they do not wish to receive any unsolicited electronic communication.

Our Web server does not collect visitor information in the form of the visitor’s domain or Internet Protocol (IP) address but does collect information regarding which pages are accessed. This information is used internally, only in aggregate form, to better serve visitors by helping us to manage our sites; diagnose any technical problems; and improve the contact of our Web site.

Following its use, the information collected is not retained but, rather, is discarded in a secure manner.

In browsing the Internet, you will encounter a technology called “cookies” which can be used to provide you with specific information from a web site as well as provide the operator of a Web site with information about you. A “cookie” is an element of data that a Web site can send to your browser, which may then store it on your system. You can set your browser to notify you when you receive a cookie, giving you the chance to decide whether to accept it. This site does use cookies but only to record session information.

St. Andrew may from time to time have a relationship with an ad server company, but does not and will not permit any ad server company to collect information about users such as domain type, IP address, and click stream information.
A user of these Web sites may access the information held by St. Andrew about them in connection with their visits to this site as well as their registration information. Users may request the correction of any inaccuracies in such information. Access to and, if required, correction of such information may be arranged by contacting the parish office at the contact information indicated below.

Evolving Practices

This Policy is in effect as of January 1, 2004. St. Andrew will from time to time review and revise its privacy practices and this Policy. In the event of any amendment, a notice will be posted in St. Andrew’s regular weekly bulletin. Policy changes will apply to the information collected from the date of posting to the bulletin as well as existing information held by St. Andrew.
